I think such holes are a norm in westerns - we all realize they have very little to do with reality. In that regard Unforgiven is closer to reality, than most.

I liked Pale Rider, when Clint goes in a Bar or Store Front full of guys with guns who want to kill him.
Several gunshots later, they are all dead, and Clint walks out.

The hero is walking slowly through the street, they show the bad guy's rifle barrel following him, long enough so some girl with a shotgun has time to discharge him.
