Why are DVDs not being released?

Does anyone really know why some movies are not even scheduled for DVD release? I figure at least three possibilities:
1. Obvious one - financial quibbling between all the distributors as to who gets what payments, etc. In other words, political hollywood stuff.
2. Reluctance to spend the capital expenses vs. the potential sales. This CANNOT be right because I see DVD sales booming and only increasing in the future.
3. Lack of production volume in replication equipment.
Who knows and can tell??
