Dual HDMI + RG6 + Ethernet Configurable Wall Plates?

Trying to put 2 HDMI, an RG6 and a CAT5 jack on one wall plate (quad?). Tried searching the net and couldn't find anything. Is this doable? Source? This is probably a long-shot but it would be great to find a recessed solution as this is going directly behind a low profile wall mount for plasma.
