Off hand the one that comes to mind is Seven, the knife suit scene; it was mostly implied IIRC. A number of people left the theater when this scene came on; I wish I would have.
My ex liked movies such as this, I never have. Never seen a Jason movie, Machete, etc., never will. Generally don't watch TV shows that have anything even remotely gory, or even killing for the most part... So hopefully my children see MUCH less than the numbers you stated.
