This is a strange and violent film. The story is based around a young woman with a sexual attraction to cars. There is a lot of nudity and simulated sex. There is more to it in terms of the mental state of lead and others she encounters.
I found the film to be in two parts. The first was involving due to the main character development. The second was not as involving after the lead took on a new identity.
In general this is a disturbing film in a number of ways. I don't recommend it.

I am gobsmacked by this getting the Palm. Where is Europe's head? I found the first half relatable because it was your quasi-typical serial killer fare. The second half really became "The Crying Game" Francophile style. I hated to see Lindon reduced to such a pathetic character. But, it was a "big challenge" he couldn't ignore, I'm sure.
Well, it won't be for the bible thumpers in the US that's for sure.However, the homoerotic thing is a bit questionable because the film is hardly that.
The reason it is a pretty great film is that it generates thoughtful criticism thematically and is visually wonderful.
This review has numerous spoilers but I think explains why it's a pretty great film.
Out of the nominated films I have seen so far I would rank Titane well above them and would have nominated it for Best Film, Best Actress, Best Actor, Best Director

All wonderful, thoughtful and filled with fine performances.
I LOVED "Drive My Car." Fantastic performances. I was moved.
I like all sorts of films and you can't spell sensationalism without the word sensational. Obviously, the voters at Cannes felt otherwise. Granted these things are highly subjective - you either like something or you don't. And I think had people read the reviews I posted they would have an understanding as to why people like it. That doesn't mean you will change your mind but they offer insightful reasons why Titane is considered very good and why many voters liked it enough to pick it as the best film. I found Titane to be a great love story the same way I found the original Terminator to be a love story. "Two people who are not determined in any ways to love each other manage to reach that level where they are really dependent and rely on each other and are everything to each other, regardless of the lies they've been telling each other when they met."
The movie is polarizing - plenty of critics have given it 4/4 stars and some have given it 0/4 stars. That was true of Pulp Fiction which won the Palme d'Or in 1994. As such, these films have no chance to win Oscars because the majority choose the safe films (Forest Gump) that tends to please the widest possible audience.
I am 35 minutes into "Drive My Car" but didn't realize it was 3 hours so I have to watch the rest later. So far it is better than the other 5 nominated films I have viewed, but I also think feel it needed another go round in editing.
So far Dune, Licorice Pizza, The Power of the Dog, Don't Look Up, West Side Story are all meh - Albeit I am not a big fan of Musicals where they seem to be highly repetitive - a fight scene (song and dance), boy loves a girl (song and dance), fight scene (song and dance) girl loves boy (song and dance), fight scene (song and dance), fight scene (song and dance), another fight scene (song and dance) one main character dies (song) - meh. Oddly, I quite like them as stage plays but not so much as movies. Plus the film has already been done so I kind of put them into a lower-tier of Academy contender as I did with A Star is Born.

I don't like films that try to shock one (unless it's a horror film, of course) into some societal opinion--- especially one that uses extreme violence and ugliness to do so. For instance, to appreciate the after effects of a rape on a character, I needn't see the most violent of rapes portrayed. A feather, in the hands of a good director, works as well as a pick axe.

I understand why it won the Palme d'Or
I quite enjoyed this rather bizarre film and agree with this film critic. (Sheila O'Malley)
I found it to be a fascinating vivid world of fantasy Body Horror/serial killer in search of something tangible.
"As full-throttle as the first half of "Titane" is, it's the second half where things really take off, where Ducournau digs deep into her subject, moving into very strange and complex waters. Alexia is not a character so much as she is the ragged wild-eyed personification of Fight-or-Flight (albeit pregnant with a baby fathered by a Cadillac)."
I never thought I'd see a film as described in the brackets.
"The language of "taking care" of one another comes up repeatedly. What does "taking care" even mean in the world of "Titane"? Everything seems so perilous and transitory! The importance of tenderness—and the pain tenderness causes to those unaccustomed to it—stands in stark contrast to the human body's inexorable processes, the body's irritating penetrability and fragility, magical titanium plate or no .../... The deeper thematic revelations may come too late in the game for those either turned off or turned on by the frenzied pitilessness of the first half, but Ducournau, inventive, bold, fearless in her approach and sensibility, doesn't lose her nerve."
gore--- to no end. Imagine Tarantino without the humor, cleverness, brilliant dialogue, directorial prowess, and catharsis. Right! Nothing is left but gratuitous violence, ugliness incarnate.
I didn't like it. I bailed out half way. I couldn't figure out why she was so violent.
I was OK with the violent aspect initially, but it just got darker and demented. Don't understand why it was so well received at film festivals in Europe.
She suffered a severe head injury. I'd guess the director did, too.
