It could be lots of things including your suggestion of transmission problems. He needs to try a few things to narrow it down. I would not suspect the HDMI cable since it works fine most of the time but it could be the HDMI interface in one of the pieces of equipment. He could try component cables to see if it makes a difference. If his audio is going to an AVR or pre/pro he could try the TV's speakers to eliminate those components. He could also try the TV with OTA transmission to see it still happens.
