For me it has always been looking under the bed and dolls in the bedroom. Poltergeist took this to a new level. The clown doll in The Game with Michael Douglas was a big one for me also.

David Lynch always creeped me out with Blue Velvet and Wild at Heart and even Eraserhead. His images and the underlying horror of his situations as well as Frank Booth were scary/creepy.

That one is the true horror movie. One feels sympathy towards the Dead who are hungry and want to eat something. Being dead, they distinguish only fellow human beings. The cant eat vegetables. That picture says a lot, that the dead are waiting for ever for a reincarnation. Or they got impatient waiting for the second coming. That movie makes us wonder about our future, our life after death.

...Halloween midnight show in college when it first came out.
Very scary.
It was preceded by a short French film that consisted of slicing an eyeball open with a razor blade. Totally turned my stomach. This set me up to be extra sensitive to the NOLD. Creepy walk back to the dorm.

Movies that come to mind that kept the suspense alive to the end:
Alien(s), Mama, The Omen (original), The Exorcist, Rosemary's Baby, the tv movie Salem's Lot directed by Tobe Hooper (still one of the best vampire movies), Blair Witch Project, Paranormal Activity (that one really pushed my scary buttons), Pan's Labyrinth...................

I am not a big fan of the hack and slash movies, although I will give the original Halloween a thumbs up. And the Scream series was pretty well done, as I liked the dark humor.And, I like something that is either original, or it has to be a very well done horror staple.
For recent examples of original horror subjects, I really liked "Splinter" and "28 Days later", (an intelligent take on how the whole zombie thing could actually work in real life).
For well done horror staples, I really liked "Drag Me to Hell" (for the standard gypsy curse horror movie), and "The Ring" was pretty good, (for the standard 'watch this video and die' premise!); and "Ginger Snaps" was pretty good (for a werewolf movie). It has been awhile since I saw a good vampire movie though, (too many of the teenage vampire movies for my taste!)
There have been a fair number of good comedy-horror movies recently, including "Shawn of the Dead" and "Fido", both of which are zombie films.
Another good film was was "Trick 'r Treat", although the humor was REALLY dark for that one!My two cents worth.

Of course having Clive Barker as the writer clearly helps!
I wish Barker would make Weaveworld into a movie.
(In fact splitting it into two movies would probably be best, so as to not cut too much as it was a fairly long book.)

Michael Haneke is a bit of a freak himself it seems
Most horror movies don't really scare me. That said, some of the Asian horror flicks from the 2000's. Most notably Ju-on, that really freaked me out. A few others spooked me to some degree: Kairo (Pulse), Ringu, Cursed, Tale of Two Sisters, The heirloom.
I should mention Uzumaki, just for being so freaking weird.
