the rear speakers is in the ceiling: anyone done that? I'm using KLH 5s for the main speakers in the front (any suggestions for those ceiling speakers would be appreciated, too).

The rears (for movies anyhow) don't do that much work other than sound effects. Had some Phase Technology rears to go with my low end Martin Logans. Kinda a pain to get them in and wired but very nice now that it is done.

Nothing I like worse about a room than a flatscreen above the fireplace or mantle. Distraction for the eyes.
while watching movies, LCD/Plasma TV mounted on top of the fireplace mantle should be fine. However, if you are sitting on a sofa then you have to consider and address the neck strain issues. Here is a guideline that will mitigate the neck strain issue while watching TV. Vertical angle of view: Where this is not possible, the SMPTE guideline suggests that the maximum vertical angle measured at the seated eye height from the front row center seat to the topmost part of the TV screen of projected image, should not exceed 35 degrees.Of course YMMV.

Not necessarily high end, but fine. I generally recommend using units with directional tweeters, but straight down can work depending where you are in relation to them. KLH does make units for this application....
Best option is something with directional tweeters though IMO.

I'd first recommend not putting your teevee at that height. The bottom edge really shouldn't be higher than 36-44" above the floor when the viewer is seated.

Agreed. I have a buddy who has his TV above the mantle (and there IS another option in his room). I find it horrible for watching. But then a dump truck and I had a disagreement years back, so I'm likely even more sensitive to viewing heights than most.

Instant neckache for me too if the screen center is above eye level, but I guess this depends on the viewer(s).
