Are you using an HD cable box?

If so, your cable box may not be properly configured. HD cable boxes have a setup menu that allows you to set the max resolution and screen size the cable box will output. I got one from the cable company that had been set to 16:9, but limited to 480i. I had to reconfigure to get higher resolution. I've also seen them set to 4:3.

You might want to investigate this.
