We have Brighthouse HD cable with a Scientific Atlanta HD box
and experience the same problems.
We had it with Cablevision when we were in NY also.
NBC is the worst offender when it comes to reliable picture
with HD. Many times we have to watch NBC in SD because NBC
is not broadcasting their HD signal correctly.
Maybe in 10 years they'll have this sorted out.
But I wouldn't hold my breath.

In Louisville, the NBC affiliate has the worst HD performance compared to the other local stations. The station management doesn't seem too motivated to get it right because they view it as a cost item only with no revenue generation. I use to complain regularly but to no avail. I get all HD locals OTA because they are not yet carried by DirecTv.

Dishnetwork had the best broadcast HD package I could get last time I checked. I don't know if anyone's one-upped them yet, but if someone had, I'd guess it would be DirecTV.
When I'm in channel surfing mode, I look at the HD channels first, then decide if I want to put up with ugly SD broadcasts to fill the big screen (the programming would have to be something I was really liked).

Your problems with HD are more complicated because you're getting the signal from a third party, the cable company. Over the Air HDTV signals directly from the broadcaster do not seem to have that many problems (assuming you can get OTA reception). The most irritating has been no center-channel audio (resulting in no dialog!), but things have been good lately.
